Medical Records will only be released with written authorization from the patient or the patient's legal representative. The Incident Report is also available upon request. All requests for copies of Medical Records must be made in writing, and faxes are not accepted. The date, time, location, and name of patient must be provided in this request. There is a $6.00 fee for copies of the medical records and incident reports. Checks accompanying the requests should be made out to the City of Newport Beach.
